Good Faith Estimate
Good Faith Estimate Notice for Clients
Wildflower Valley Therapy
Licensed Clinical Social Worker (LCSW)
Notice to Clients and Prospective Clients
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under the law, health care providers must give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for services.
What You Should Know:
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes therapy sessions and other mental health services.
The estimate will be provided in writing, typically at least one (1) business day before your scheduled service or upon request.
If you schedule a service more than 3 business days in advance, the estimate must be provided within 1 business day.
If you schedule a service 10 or more business days in advance, the estimate must be provided within 3 business days.
You may request a Good Faith Estimate before you schedule a service, and we will provide it within 3 business days.
What’s Included in a Good Faith Estimate?
Your Good Faith Estimate will include:
The type and frequency of services you are scheduled to receive (e.g., individual therapy, 50-minute session)
The provider's name and credentials
The expected cost per session
The estimated total cost for a period of treatment (e.g., weekly sessions over 3 months)
Disclaimers about variability of treatment needs and duration
Dispute Resolution:
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be eligible to dispute the bill.
You can initiate a dispute resolution process through the U.S. Department of Health and Human Services (HHS). There is a small fee to start this process.
